Optical Mark Recognition or OMR is a technology that can read dark marks made on specially prepared sheets, also called OMR sheets. These sheets have four index points on its corners due to which the OMR software can read the dark marks made on the bubbles of the sheets. While the OMR answer sheets are the most popular among OMR sheets (as they can be checked quickly and accurately), OMR forms are also used for several reasons.
The OMR forms are commonly used as admission forms, application forms, etc. as it is easy to collect and process the data of the students or candidates. Apart from this, many organizations, companies and institutes have started using OMR forms for feedback and surveys too. This is because surveys and feedbacks have lot of participants and analyzing each form would be very time consuming. Hence, most organizations consider OMR feedback and survey form the best option.

Printing these OMR sheets is quite easy. You do not need any special printer for this purpose. Any inkjet or laser printer can be used for printing OMR feedback and survey forms. You just have to make sure that the index points are printed perfectly (they should be dark).
Bubbles must be filled completely; otherwise the OMR reader software won’t be able to collect that information.
Fill the sheets using ball-point pen only, as markers, ink pens and gel pens will cause blotting.
Handle the sheets with care; never fold or crumble them.
