The application form, also sometimes known as the admission form, is a part of every school and college admission procedure. While most educational institutions use handwritten application forms for their admission procedures, many of them are opting for OMR application forms. This is because most schools and colleges have to process hundreds and sometimes thousands of application forms every year. Processing them manually can be time consuming and requires huge man power too. On the other hand, the OMR software reads and processes the sheets quickly (3 sheets in 1 second, to be exact).
Here we present a sample OMR application form for schools designed by Addmen using the OMR software. This sample application form is used by a school for their admission procedure.
This application form begins typically with blocks for name, father's name and last name of the student.
This is followed by roll no., gender, date of birth, mobile no. 1 and parent mobile number.
All these fields have blocks for written text as well as OMR bubbles.
Candidates or their parents have to write the text for each block and also fill the OMR bubbles accordingly.
At the bottom of the page, there are two separate blocks where the candidate has to provide the necessary information.
The first block asks for fee related information like total fees, paid fees, fees paid date, receipt number, concession fees amount and comments.
The second block asks for information like mother's name, school/college name, class, email ID, parent's email ID and permanent address. Although the OMR software cannot read these blocks for written text, it can capture the images and copy it in the Excel sheet.
