Assessment forms are used in schools, colleges and offices to evaluate or analyse the skills of a person. Students can be given assessment forms to know how much they have understood in every subject. In the same way, employees in various organizations are also given assessment forms to understand how much they know about a particular product or service that the company offers.
Although the purpose of OMR answer sheets and OMR assessment forms is the same, these usually look quite different.
The OMR answer sheets only have OMR answer blocks for all the questions, while the questions are provided in a different sheet.
In case of OMR assessment forms, the questions are usually in the same sheet.
Also, the questions in OMR assessment forms may be in text format as well as in the form of pictures and diagrams.
Here we have presented an OMR skill assessment form that has been used to assess the technical skills of the employees of an organization.
The beginning of the OMR assessment sheet has similar elements as the OMR answer sheet. In place of roll number and question paper ID, candidates have to fill in their Form ID, Dealer Code and ASC Code.
They also have to mention their designation, shift and duration.
The next block is where they have to fill details like name, name of the dealer and location, ASC branch name and location and date.
There is a section to note down the marks obtained by the candidate.
There are 11 questions in total in this sheet. Each question is followed by diagrams, usually in the form of answer options.
Candidates have to read the question, check out the images or diagrams and then mark the right option.
The OMR assessment forms do not have to be contained in one single page. It can be of as many pages as required.
