The HR, team leaders and managers are responsible for providing employee feedback every quarter, in six months or every year. There are two sections in this employee feedback OMR sheet.
The first section focuses on the leaves taken by the employee as well as his or her performance at work. The feedback has to be provided for the last six months.
The first section has four sub sections and each of these is allotted 15% weightage. In other words, the first section of the feedback form holds 60% weightage.
The second section also has four sections and holds 10% weightage each, providing the section a total weightage of 40%.
The scoring pattern of both sections is also different. While the parameters of the first section are in the numerical form, the parameters of the second section are poor, needs improvement, average, above average and excellent.
This OMR form has a barcode as well as space for the HR or team manager to write the employee's name.
