The Emergency Health Card, as the name suggests, is a card that can be used during emergency situations. In case a person meets with an accident, he/she may not be in the position of conveying his/her personal details. In such situations, the Emergency Health Card would be quite useful. This card contains all the important information about the person like name, blood group, allergies, health problems, emergency contact numbers, etc. Here we have provided an OMR survey form that tries to know the opinions of the working youth about the concept of Emergency Health Card.
The OMR sheet begins with the details like surveyor code, location and date of survey. There is space to not only write these details but also to fill the OMR bubbles.
Although the candidates do not have to mention their personal details (name, address, mobile number, etc.), they have to mention things like gender, age and resident city.
Instructions for filling the sheets are also mentioned. The candidates have to rate some of the questions in the range of 1 to 5. 1 means strongly disagree and 5 means strongly agree. 2, 3 and 4 are disagree, neutral and strongly agree, respectively.
