Manufacturers of FMCG (Fast Moving Consumer Goods) products conduct surveys before or after launching their products in the market. This helps them to understand what consumers want, what is popular, whether their product has gained popularity, etc. This feedback helps them to plan the promotion of the product or come up with a better product. Here is an OMR survey form used by a leading FMCG goods company for market survey of its products.
This survey is a part of a promotional activity held for Yippee noodles. This survey is taken in Bangalore city.
The instructions for filling the form are provided. These have to be followed by the promoter. Instead of a tick mark or a fully filled bubble, the promoters are asked to fill the box with an X mark. Moreover, this mark should not run out of the box.
The promoter should not use pencil for filling the form. Only pen should be used for this purpose.
The promoter has to write his/her name in the space provided for this purpose. This is followed by the name of the supervisor, location, date on which the survey was conducted, start time and end time.
There are six small forms in this OMR survey form. Each form asks for the name of the customers and their numbers. The mobile number has to be mentioned in OMR format too.
The customers are asked about the current brand of noodles. The options provided are Maggi, Yippee, Top Ramen and Other. They are also asked whether they know about the product.
The customers have to mention the number of units of noodles they purchased. Other that this, they also have to provide the reason for not purchasing Yippee noodles.
