Questionnaires are provided to conduct surveys about a particular product, survey or issue. It is also a great way of taking feedback from a group of people. As a large number of people participate in these surveys, checking and assessing the sheets manually can be problematic and time consuming. This is the reason why several organizations prefer to use the OMR questionnaire form.

The OMR questionnaire provided here is divided into four sheets. These samples of OMR based questionnaires have been used for survey of voters for awareness about election procedures. These sheets are printed in Hindi; however, these can be printed in any language you desire, as the OMR software focuses on the bubble marks and not on the words.
These OMR questionnaires are dedicated towards knowing the knowledge of a group of people about the election and voting process.
The first sheet is dedicated towards voter survey.
The second is about the voting process.
The third sheet has questions about the election process
Fourth sheet is about voter card awareness.
Once the forms are filled by the people, they are scanned and then read by the OMR form reader software. The software extracts the data from the sheet and presents it in the Excel sheet.
