All Educational institutions aim to provide good quality education to its students. Most institutes look for ways and means to improve the quality of their course, syllabus, staff and their teaching techniques, by taking anonymous feedback from the students. Students are asked to provide their honest opinion about the course they have taken and also about their teachers or professors.
Optical Mark Recognition or OMR sheets are preferred for designing feedback forms as processing the sheets after the students have filled them is easy. You do not have to manually check them individually. The OMR reader software reads the OMR feedback forms and provides the data in Excel sheet format.

The instructions section states that the details of the sheet would be shared with the faculty but only after grade submission.
This OMR feedback form is meant for one course and one instructor. The students have to mention these in the spaces provided for the purpose.
The students also have to write their roll no. and fill the OMR bubbles for the same.
The OMR feedback form is divided into four sections. Section A contains six questions with respect to 'teaching content and pedagogy.
Section B asks about 'evaluation' done by the teachers (includes four questions), while Section C asks about the 'professional conduct' of the faculty members (includes three questions).
The students have to rate each parameter on the scale of 1-5, where 1 stands for “complete disagreement” and 5 for “complete agreement”. They can also mark NA if a particular question is not applicable in their situation.
Section D is where students can provide their comments. They can write briefly about their experience with the instructor or during the course.
